What Is an NDIS Mid-Term Audit?
A mid-term audit is a mandatory off-site review conducted partway through your three-year certification cycle, due 18 months from your registration date. It assesses whether you are continuing to meet the NDIS Practice Standards and Quality Indicators that applied at your initial certification audit — not whether you meet a new or different standard.
Unlike your initial certification audit, the mid-term audit is generally less intensive and does not always require an on-site visit. It still involves a genuine assessment of your operations: a review of your policies, procedures, and participant files, combined with interviews of staff and participants who consent to take part.

What the Mid-Term Audit Assesses
The mid-term audit focuses on the Core Module of the NDIS Practice Standards — the requirements that apply to every certified provider regardless of registration group. This includes:

Whether your organisation continues to operate with clear accountability and an active, applied risk management approach.
Participant rights & engagement
Whether participants are still receiving person-centred, rights-based support consistent with what was demonstrated at your initial audit.
Incident management & safeguards
Whether your incident reporting, complaints handling, and safeguarding practices remain current and are being applied consistently.
Quality systems
Whether your continuous improvement processes are active, and whether any non-conformances from your initial audit have been properly addressed.
If your initial audit identified non-conformances, your mid-term audit will check that the corrective actions you committed to have actually been implemented.
Why the Mid-Term Audit Matters
A mid-term audit is a condition of your registration, not a formality. Failing to complete it by the due date, or failing to demonstrate continued compliance, can result in your certificate being suspended, withdrawn, or cancelled — which directly stops your ability to deliver NDIS-funded supports.
Providers sometimes underestimate the mid-term audit because it is described as “less intensive” than the initial certification audit. That description refers to scope, not stakes. An auditor who finds your incident register has gone quiet, your policies have not been updated, or your staff cannot describe your complaints process will record those findings the same way an initial auditor would.

When to Start Preparing
Most providers should begin mid-term audit preparation three to four months before the due date. This gives enough time to identify and close gaps properly, rather than rushing remediation in the final weeks. If your initial audit raised any non-conformances, start earlier — auditors will specifically check that those issues have been resolved, and rushed fixes are often the ones that do not hold up.
